Transfer Students
The Admissions Committee reviews each application individually, considering a combination of factors before making a decision. Please follow the appropriate procedures below to ensure the timely review of your child’s application:
Please Note: All requested documents must be turned in for the file to be reviewed by the admissions committee.
Documents needed for all transfer Students 9-11th Grade:
- Admissions Application
- Last FCAT or Standardized Test
- Most Current Report Card
- Official Transcript (in a sealed envelope with dry seal)
- School Evaluation Form
- Record Release Form
- Initial & Most Recent Evaluation Form (ESE students only)
- I.E.P. (ESE students only)
- Copy of Birth Certificate
- Social Security Number
- Parish Letter, if applicable
- Original Immunization Form
- Original Health Form
